What VA Form 21-0781 is for.
VA Form 21-0781 asks about the traumatic event itself: what happened, roughly when, where, what unit you were with, and who else was there. VA uses the answers to try to corroborate the event in service records — which is why the dates and the unit matter as much as the description.
Since June 28, 2024 there is one form for every kind of stressor. The old 21-0781 covered combat and other stressors; a separate 21-0781a covered personal assault and military sexual trauma. The 2024 revision folded both into a single 21-0781, and the 0781a was discontinued. Any guide that still tells you to file a 0781a is out of date.
The form also lets you point VA to markers — behavior changes, requests for transfer, a drop in performance, medical visits — that can corroborate an event nobody reported at the time. For MST and assault claims, that's often the whole case.

When VA Form 21-0781 is the right form — and when it isn’t.
This is the form you need if…
- You are claiming PTSD, or another mental-health condition (depression, anxiety, an adjustment disorder) that a clinician has tied to an event in service.

A month and year, a base or a location, and the unit you were assigned to. VA searches unit records and deployment histories for corroboration, and the search needs a window to look in.
Name anyone who was there or who you told
Fellow service members who witnessed it, a chaplain, a friend, a family member you called afterward. Each is a possible buddy statement on the 21-10210.

Leaving the dates blank because you don't remember exactly
An approximate window — "spring 2008, Camp Leatherneck" — gives VA somewhere to search. A blank gives it nowhere. Approximate is fine; say that it's approximate.

Nightmares and hypervigilance belong on a 21-4138 or in your clinician's notes. The 0781 is where the event goes; that's the thing VA is trying to establish.
