Notes on this code
These notes are part of the regulation. They change how the levels above are applied.
- Note: This diagnostic code includes short bowel syndrome, mesenteric ischemic thrombosis, and post-bariatric surgery complications. Where short bowel syndrome results in high-output syndrome, to include high-output stoma, consider assigning a higher evaluation under DC 7329 (Intestine, large, resection of).
Small Intestine Resection (VA Diagnostic Code 7328)
Digestive System · 38 CFR § 4.114
Understanding This Condition
A small intestine resection is a surgical procedure in which part of the small intestine is removed. Under VA Diagnostic Code 7328, the effects of this procedure are evaluated based on how they affect nutrition, digestion, and daily functioning.
This diagnostic code covers several related conditions, including:
- Short bowel syndrome
- Mesenteric ischemic thrombosis
- Post-bariatric surgery complications
When rating this condition, the VA focuses on symptoms such as diarrhea, undernutrition, anemia, and the level of medical or nutritional support a person requires.
Available Rating Levels
The VA assigns a disability rating based on the severity of symptoms and the type of treatment needed:
| Rating | What It Reflects |
|---|---|
| 80% | Status post intestinal resection with undernutrition and anemia; and requiring total parenteral nutrition (TPN). |
| 60% | Status post intestinal resection with undernutrition and anemia; and requiring prescribed oral dietary supplementation, continuous medication, and intermittent total parenteral nutrition (TPN). |
| 40% | Status post intestinal resection with four or more episodes of diarrhea per day resulting in undernutrition and anemia; and requiring prescribed oral dietary supplementation and continuous medication. |
| 20% | Status post intestinal resection with four or more episodes of diarrhea per day. |
| 0% | Status post intestinal resection, asymptomatic. |
What the VA Looks For
When evaluating a small intestine resection, the VA considers:
- Frequency of diarrhea — specifically, whether there are four or more episodes per day.
- Undernutrition and anemia — whether the resection has led to these nutritional and blood-related complications.
- Level of treatment required — including prescribed oral dietary supplementation, continuous medication, and total parenteral nutrition (TPN), whether intermittent or total.
- Symptom presence — whether the condition is symptomatic or asymptomatic.
Important note: Where short bowel syndrome results in high-output syndrome, including high-output stoma, a higher evaluation may be considered under Diagnostic Code 7329 (Intestine, large, resection of).
