# DC 7705 - Immune thrombocytopenia:

Rated under § 4.117. Body system: Hemic and Lymphatic Systems.

## Rating criteria

| Rating | Criteria |
| --- | --- |
| 100% | Requiring chemotherapy for chronic refractory thrombocytopenia; or a platelet count 30,000 or below despite treatment |
| 70% | Requiring immunosuppressive therapy; or for a platelet count higher than 30,000 but not higher than 50,000, with history of hospitalization because of severe bleeding requiring intravenous immune globulin, high-dose parenteral corticosteroids, and platelet transfusions |
| 30% | Platelet count higher than 30,000 but not higher than 50,000, with either immune thrombocytopenia or mild mucous membrane bleeding which requires oral corticosteroid therapy or intravenous immune globulin |
| 10% | Platelet count higher than 30,000 but not higher than 50,000, not requiring treatment |
| 0% | Platelet count above 50,000 and asymptomatic; or for immune thrombocytopenia in remission |

## Notes

- Note (1): Separately evaluate splenectomy under diagnostic code 7706 and combine with an evaluation under this diagnostic code
- Note (2): A 100 percent evaluation shall continue beyond the cessation of chemotherapy. Six months after discontinuance of such treatment, the appropriate disability rating shall be determined by mandatory VA examination. Any reduction in evaluation based upon that or any subsequent examination shall be subject to the provisions of § 3.105(e) of this chapter

# Immune Thrombocytopenia (VA Diagnostic Code 7705)

## Understanding Immune Thrombocytopenia

Immune thrombocytopenia is a condition affecting the blood, evaluated by the VA under Diagnostic Code 7705 within the Hemic and Lymphatic Systems. The VA assigns a disability rating based largely on your platelet count, the treatments your condition requires, and any related bleeding or complications.

Because these factors can change over time, the VA's rating criteria span several levels — from a fully symptomatic, treatment-dependent condition down to a condition in remission.

## Available Rating Levels

The VA uses the following rating schedule for immune thrombocytopenia:

| Rating | What the VA Looks For |
|--------|------------------------|
| **100%** | Requiring chemotherapy for chronic refractory thrombocytopenia; **or** a platelet count of 30,000 or below despite treatment |
| **70%** | Requiring immunosuppressive therapy; **or** a platelet count higher than 30,000 but not higher than 50,000, with a history of hospitalization because of severe bleeding requiring intravenous immune globulin, high-dose parenteral corticosteroids, and platelet transfusions |

| **10%** | Platelet count higher than 30,000 but not higher than 50,000, not requiring treatment |

## What the VA Considers

When rating immune thrombocytopenia, the VA reviews:

- **Your platelet count** — a central measure across every rating level.
- **The treatment your condition requires** — such as chemotherapy, immunosuppressive therapy, oral corticosteroids, or intravenous immune globulin.
- **Bleeding and complications** — including mucous membrane bleeding and any history of hospitalization for severe bleeding.

## Important Notes

- **Splenectomy is evaluated separately.** If you have had a splenectomy, the VA evaluates it under Diagnostic Code 7706 and combines that evaluation with your rating under Diagnostic Code 7705.
- **100% ratings and chemotherapy.** A 100 percent evaluation continues beyond the cessation of chemotherapy. Six months after such treatment is discontinued, the appropriate disability rating is determined by a mandatory VA examination. Any reduction in evaluation based on that or any subsequent examination is subject to the provisions of § 3.105(e).
